II. Pharmacological properties

7. Novocaine belongs to the group of local anesthetics.

Novocaine blocks sodium channels, displaces calcium from receptors located on the inner surface of the membrane and, thus, prevents the generation of pulses in the endings of the sensory nerves and impulses along nerve fibers. Suppresses carrying out not only pain, but also impulses of another modality. With intravenous administration it has a general effect on the animal’s organism, reduces the formation of acetylcholine and reduces the excitability of peripheral cholinergic systems, has a blocking effect on vegetative ganglia, reduces spasms of smooth muscles, lowers the excitability of the heart muscle and the excitability of the motor cortex of the cerebral cortex. In toxic doses causes excitation, then paralysis of the central nervous system.

In the body, it rapidly hydrolyses, forming para-aminobenzoic acid and diethylaminoethanol, which are pharmacologically active substances.

After administration, the drug acts quickly and briefly. In terms of the effect on the organism Novokain 0.5%, 1%, 2% solution for injection refers to substances of low hazard (hazard class 4, Novocaine powder – substances highly hazardous (hazard class 2) according to GOST 12.1.007-76.

III. The order of application

8. Novocaine 0.5%, 1%, 2% solution for injection is used, if necessary, previously diluted with sterile saline to the desired concentration, for infiltration anesthesia in the form of 0.25% -0.5% solution; for anesthesia by the method of A.V. Vishnevsky (tight creeping infiltration) – 0,125% -0,25%; for conductor and spinal anesthesia 1% -2%.

In ophthalmology, 0.5% solution of novocaine is used for keratitis, keratoconjunctivitis, periodic inflammation of the eyes in horses (infraorbital blockade).

In the obstetric-gynecological practice, solutions of novocaine are prescribed for endometritis, metritis, prolapse of the uterus and vagina, detainment of the afterbirth in cows and goats (paranephalic blockade according to AV Vishnevsky), with serous-catarrhal mastitis (blockade of the udders of the udder according to BA Bashkirov or DD Logvinov) in the form of a 0.25% -0.5% solution.

Novocaine 0.5%, 1%, 2% solution for injection is also used as a solvent for medicines.

9. Contraindications to the use of Novocaine 0.5%, 1%, 2% solution for injection is not established, except for individual hypersensitivity to procaine.

10. Novocaine solutions are applied subcutaneously (0.25% -0.5%), intramuscularly (1% -2%).

The concentration of the solution, the dose, the method of application depends on the nature of the surgery or the course of the disease, the type, weight, age of the animal and its condition.

With local anesthesia, the toxicity of novocaine is higher, the more concentrated the solution is. In connection with this, with increasing solution concentration, the total dose is reduced or diluted standard solution of the drug to a lower concentration (0.125% -0.25%) with sterile isotonic sodium chloride solution 0.9% or Ringer-Lock. These dilutions are prepared immediately before use.
